A funny image but worthwhile sharing as it does nicely reflect what I regularly saw.

I guess my most valueable comment would be for ALL management levels to engage in regular, upfront and honest discussion with all layers of the company.

Hidden issues out there, those you may ignore, can really block progress and at the end be an underestimated cost factor, too.
